We specialize in Customized Living (CL) and a wide range of Home and Community-Based Services (HCBS) tailored to meet each client’s unique needs.

Customized Living (CL)

Customized Living (CL) is a coordinated package of regularly scheduled, health-related, and supportive services. These services are available to individuals aged 55 or older residing in:

  • Their own private homes
  • Public housing buildings
  • Section 42 or Section 811 housing communities

Our CL services are funded through the following Minnesota waiver programs:

  • Brain Injury (BI) Waiver
  • Community Access for Disability Inclusion (CADI) Waiver
  • Elderly Waiver (EW)

24-Hour Customized Living

We also offer 24-Hour Customized Living, which provides around-the-clock care and supervision to clients who require continuous support due to medical or personal needs. This service ensures a safe and secure environment for individuals needing enhanced monitoring and assistance at any time of day or night.

Component Services

Our CL program includes a broad range of health-related and supportive services such as:

  • Medication setup and monitoring
  • Personal hygiene and grooming assistance
  • Daily meal preparation and nutritional support
  • Laundry and housekeeping
  • Mobility and transfer assistance
  • Behavioral and emotional support

Home and Community-Based Services (HCBS)

Beyond Customized Living, we also offer essential HCBS designed to help individuals remain in their homes and actively participate in their communities. These include:

  • 24-Hour Emergency Assistance – Immediate help in case of crises or urgent needs
  • Homemaking and Cleaning – Support with household tasks to maintain a clean and safe environment
  • Individual Community Living Support (ICLS) – Help with daily living activities to promote independence
  • Individualized Home Support (IHS) – Without Training – Supervision and assistance without formal skill development
  • In-Home Respite Care – Temporary relief for family caregivers while ensuring the client receives attentive care
  • Night Supervision – Overnight assistance and monitoring to ensure safety and support during nighttime hours
